The History of Community Living Connections
Community Living Connections, Inc. is a local, non-profit agency founded in 2004. CLC currently provides community supported living services to approximately 200 individuals who live throughout Madison, Stoughton, Green Bay, Platteville, Beaver Dam and Fond du Lac.
About CLC’s Community Supported Living Services & Supportive Home Care Services
Our organization provides support that enables people to live in their own homes and participate in community life. We believe that individuals, regardless of ability, deserve equal opportunities to create their "best life"; they just need additional support in order to do so. Community Living Connections provides in home staff to help with planning, personal cares, medical support, finances, relationship building…etc. We work in partnership with people throughout their lifetime.
The success of our organization is measured by the individual successes of the people we serve. Adults with developmental disabilities living their dreams, traveling, buying their own homes, getting married, working, owning a business - choosing and directing their own lives. Successes that are helped made possible by the dedication and commitment of the people who work here.
About CLC’s Corporate Guardianship Services
CLC provides Guardianship of Person and Guardianship of Estate services, primarily for adults who experience developmental disabilities and frail elders. These services are available in 12 Wisconsin counties: Brown, Columbia, Dane, Dodge, Fond du Lac, Grant, Green, Iowa, Jefferson, Lafayette, Rock, and Sauk.
Our goal is to provide consistent and thoughtful decision-making for the wards CLC is appointed guardianship over. CLC guardians will seek to develop strong relationships with the individuals and their support networks and to include the person in life decisions whenever appropriate, so that outcomes are based in the person’s own values & goals.
